How many regions should we include?

No. The one thing about PoKéQuesT is that it has to hold true to the canon world.

I would include only Johto, Kanto and the islands, but that's just me. A major problem with PDoA in my opinion is way too many regions, so I would not put so many in if I were making a game.

Yeah, I was talking with my friend and for the first alpha version just Kanto.. but it would really suck if the final version didn't have hoenn and sinnoh & al.

For starters I would suggest just the main regions and the orange islands.
